date selector not working in front-end form view

date selector not working in front-end form view was created by TechSum
I have a date selector on a front-end form and nothing happens when clicking the selector. If I'm creating a new entry from the admin form the date selector works fine. But on the client form it doesn't do anything. The only error I see in the browser console is the following:
