Method to get the username remind request form.

Signature

public function getForm(array $data, [boolean $loadData = true] )

Parameters

$data

An optional array of data for the form to interogate.

$loadData

True if the form is to load its own data (default case), false if not.

Returns

JForm

A JForm object on success, false on failure
